                            Consent to Participate in the Parenting Initiative



                 This consent form applies to:       Name: _________________________________________
                 The following information is provided to inform you about the Citizen Security and Justice

                 Programme III (CSJP) Parenting Initiative.  You will be given an opportunity to ask questions,
                 and have your questions answered.  In addition, you will be given a copy of this consent
                 form.

                 1.  Purpose of the initiative.  This initiative is being conducted by CSJP in order to better

                    understand the experience of parents with raising children in Jamaica today and to
                    identify ways in which to help them with the challenges they may face.


                 2.  Selection of families for participation in the initiative and description of procedures

                    to be followed. There will be over 600 families participating in this initiative. You may
                    choose whether or not you want to participate. If you choose to participate, then
                    during the next six months, CSJP will keep in regular contact with your family, along
                    with all the other participating families. Also during the next six months, some families

                    may be asked to attend workshops, allow persons working with the CSJP initiative to
                    meet  with  family  members  in  the  home  and  participate  in  different  family  related
                    activities.


                 In addition, all families are being asked to complete a basic data form now, capturing
                 basic information about the household and family members. This form is to be completed
                 by the parent/guardian/caregiver with primary responsibility for the care of the child. The

                 family will also be required to complete a follow up survey in six months and again in twelve
                 months’ time. At that time, a survey firm will contact you to schedule an appointment to
                 conduct the survey. All parents will be interviewed in their homes to ensure that all persons
                 are interviewed in a similar place at first. This is just the requirement of the survey you will
                 participate in. There are no right or wrong answers to any of the questions in the surveys;

                 we are interested in your own opinions and experiences which will help us understand the
                 experiences of parents with raising children in Jamaica today.



                 As the initiative involves the whole family, the follow up survey will be done amongst one
                 parent/guardian in the home and up to three eldest children between the ages of 6 – 15
                 years.
